Prior to your first visit

Please go to the Downloads page and download the following forms and complete them before your first visit. The naturopathic intake package is thorough, as it is important to have a full health history in order to formulate a comprehensive treatment plan for you. The forms will take about 15 minutes to fill out.

Your first visit (75-90 minutes) –
$150 + HST (adults); $135 + HST (children under 18) The initial visit consists of an in-depth review of your health concerns and goals, as well as a naturopathic assessment. The information gathered will be used to create an individualized wellness plan for you.

Your follow-up visits (30-45 minutes) –
$75 + HST (adults); $65 + HST (children under 18) Follow-up visits are scheduled to monitor your progress and, if necessary, to refine and expand your plan of care. As you notice improvements in your health, the focus will turn to health maintenance through the development of a personalized prevention plan.

The frequency of your visits will depend on the pace of your progress, the type of therapy you choose and the amount of support you need to implement your plan of care.
